Yes. A referral is required to schedule your first appointment. You or your doctor's office can call us at 302-280-6358 to start the process. We'll walk you through what's needed and get you scheduled.
Call our office at 302-280-6358 during business hours — Monday through Friday, 8:00 AM to 4:00 PM. Either you or your doctor's office can make this call. Have your referral and insurance information ready if possible.
Please bring the following to your first visit:
- Valid photo ID
- Insurance card (primary and secondary if applicable)
- List of current medications, including dosages
- Referral documentation from your provider
- Any relevant medical records or prior imaging
- Payment — due at time of service

Medication-Assisted Treatment (MAT) is an evidence-based, FDA-approved approach to treating opioid use disorder. It uses medications like Suboxone (buprenorphine/naloxone) to reduce cravings and withdrawal symptoms, making it easier to focus on recovery and daily life.
MAT is not "replacing one drug with another" — it is a medical treatment, just like medication for any other chronic condition. At Four Peaks Medical, MAT patients receive the same compassionate, structured care as all our patients.
Suboxone is a brand name for buprenorphine/naloxone — the most commonly used medication in MAT for opioid use disorder. It works by binding to opioid receptors in the brain to reduce withdrawal and cravings without producing a significant "high."

No. Opioid use disorder is a medical condition. Seeking help is not a weakness — it is the right thing to do. At Four Peaks Medical, MAT patients are treated with the same dignity and respect as every other patient. Our practice was built around this belief.

Our current self-pay rates are:

- Follow-Up Visit$150
- Medication Management$125 – $150
- MAT Program$175
- Urine Drug Screen$25 – $50

Yes. Virtual appointments are available for eligible patients. You'll need a device with a camera and microphone and a stable internet connection. You must be located in Delaware at the time of the visit. Call us to find out if telehealth is right for your situation.
Telehealth coverage varies by insurance plan. Many plans do cover virtual visits. Call our office at 302-280-6358 and we can help clarify your coverage before your appointment.
We are located at 116 E. Front Street, Suite A, Laurel, DE 19956. Office hours are Monday through Friday, 8:00 AM to 4:00 PM, by appointment only. Telehealth appointments are also available for eligible patients.
