Four Peaks Medical is actively accepting new patients. Here's everything you need to know before your first visit.

A referral is required to become a patient. You or your doctor's office must call our office to schedule. This helps ensure we can provide you with the structured, appropriate care you need.

How to become a patient

The process is straightforward. Call our office, and we'll take it from there.

Call 302-280-6358 during business hours — Monday through Friday, 8 AM to 4 PM. You or your doctor's office can make this call.

Provide your referral

Have your referral documentation ready. This can come from your primary care doctor, a specialist, or can be self-referred in some cases — ask us when you call.

Schedule your appointment

We'll schedule your first appointment and let you know exactly what to bring and what to expect. Both in-person and telehealth options are available.

Attend your first visit

Come prepared with your documents (see below). Your first visit is a comprehensive new patient evaluation where we review your history and build your care plan.

What to expect from your care

Four Peaks Medical takes a whole-person approach to pain. That means understanding your history, your goals, and what's getting in the way of living the life you want. We build plans that are realistic, structured, and designed to help you improve — not just manage.

For MAT patients: you will be treated with the same respect and dignity as any other patient. Opioid use disorder is a medical condition, and seeking help is a sign of strength.

Managing your care online

Four Peaks Medical uses Tebra as its electronic health record and patient portal. Once you are an established patient, you can access your records and manage your care through the Tebra portal.
